<note>Maternal Mortality Rate (MMR) is an indicator to see maternal health efforts. MMR
is the ratio of maternal mortality during pregnancy, childbirth and childbirth caused
by pregnancy, childbirth, and the puerperium or its management but not due to
other causes such as accidents or falls in every 100,000 live births. Based on the
results of Supas 2015, in general, there was a decrease in maternal mortality
during the period 1991-2015 from 390 to 305 per 100,000 live births. However, the
downward trend in maternal mortality has not succeeded in achieving the MDGs
target of 102 per 100,000 live births in 2015. Results in 2015, the maternal
mortality rate was three times higher than the MDGs target. The goal of this
continuous care midwifery care is to provide continuous care starting from
pregnancy, childbirth, postpartum and family planningThis research method is a
description, the form of case research, the subject of pregnant women, Mrs. M.R, 20
years of gestation, 37-38 weeks at Puskesmas Suwawa. The technique of data
testing is by observation and observation (observation) and using field data
analysis techniques. The instruments used in this study were the midwifery care
format, new interviews, examination checklists, examination tools and the MCH
HandbookThe results showed that the care given to Mrs. M.R, age 20 years starting
from pregnancy, childbirth, childbirth, and newborns running normally, the
condition of the mother and baby are in good health. In conclusion, the continuity of
care provided by midwifery from pregnancy, childbirth, childbirth, and newborns
can benefit mothers and babies in the process. Continuity of midwifery care can
optimize detection of maternal and neonatal high risk</note>
